BreakThru™ Connect Calls #
When turned on, specially permissioned caregivers can cause the care recipient’s phone to automatically answer call with the speakerphone enabled and camera turned on;
Caregivers can remotely change between the care recipient’s front and back video cameras
When to use:
Helpful both for potentially urgent situations or to assist a care recipient with video calls
Default setting:
On; Care recipient phone must be Android-based for this feature

Automated follow-up notifications when caregivers are looking into situations or resolving issues
When to use:
When looking into care alerts, these help keep everyone on care team up-to-date and on same page
Default setting:
All care team members setup to receive a particular alert type will automatically get related follow-up- care coordination notifications

Embedded video calling makes it easy for care recipients and caregivers

When to use:
Use for checking-in; ability to remotely control camera may be helpful if care recipient has difficulty with video calling

On; Care Recipient phone can be either Android or iPhone
REMOTE CARE #
Tools to help caregivers engage with care recipients remotely
Destination Checks & Appointment Reminders #
Destination Checks detect arriving or not arriving at intended appointments, events or other destinations based on GPS readings;
Optionally send an accompanying reminder to attend to the care recipient;
As an example, help see the care recipient gets home safely at night
When to use:
To help know whether care recipient made it a regularly visited location at the usual time (like work, volunteer, school, activities, friends or back home);
Also, use to help know whether care recipient made it to a particular appointment, activity or event.
Can optionally setup a pop-up reminder for the care recipient to be there as well.
Default setting:
A default “Back Home” Destination Check is setup for 10:00 PM. This can be changed, inactivated or removed.

Medicine Reminders #
Automated medicine reminders;
Medicine reminders group together those medications that are scheduled to be taken at the same time within a single reminder;
Each medicine reminder provides prescription details (if needed) and allows for meds to be confirmed all together or declined individually;
Medicine reminders expire after about 3 hours to avoid taking at the wrong time
When to use:
Suggest using if care recipient finds it helpful for remembering when to take which medicines

In case of concern that phone is with a non-permissioned user, the care recipient or permissioned caregivers can remotely log out of Care Recipient’s LeapThru® app (LeapThru® VIP for Care Recipients) via the LeapConnect™ portal (requires Care Recipient’s phone has a live connection to the Internet).

Medical Profile #
A high level medical profile can be optionally filled out with emergency information, caregiver contacts and a person’s medications, medical conditions and allergies.
The Medical Profile is made available both on the LeapConnect™ portal and the LeapThru® VIP™ phone application.
When to use:
- The profile can be made available for permissioned members of the care team to use for internal reference;
- The profile can also be used in emergency situations as a reference for ambulatory services or emergency care.
If used to make available for emergency personnel, you may want to add a sticker or other label on the phone that states the medical profile is available with instructions for accessing. This also would mean user may turn off password protection to the phone for easier access – which will expose any user input medical data as well as other personal and wellness information in the care recipient’s profile to anyone who can access the phone.

No medical data is input by default (unless LeapThru LeapConnect is delivered through our health partners for particular health programs);

Ordering 3rd Party Services #
Care recipients and permissioned caregivers can arrange transportation, meals, care visits, pharmacy refills, event sign-up and having other items delivered in one place with point-and-click ease* (currently available only through health partners)
When to use:
- Integrated 3rd party services to make it easier and faster for care recipient or caregivers to order services;
- Preferences are saved for quick ordering
- Shared services fund from which services are paid for makes it easy for care recipient or care team members to contribute or pay on behalf of their care recipient
Default setting:
This feature is currently available only through connected LeapThru health partners
Remote Reminders #
Wellness Checks #
Customizable check-ins that enable point-and-click engagement with care recipient in order to:
- > Allow care recipient to indicate a need for assistance;
- > Monitor level of pain, discomfort, or chronic symptoms over time; and
- > Provide to-do tasks to complete related to exercises, therapy or chronic programs
Concerning responses can be setup to notify permissioned care team members.
When to use:
- > If you want to be alerted if care recipient’s pain, swelling or other indicator seems high or requires attention;
- > If you want to be alerted for episodic symptoms from chronic issues;
- > As way to automatically check-in on a regular basis (eg. every morning) to see how someone is doing or as a reminder to complete certain tasks, activities, or exercises and get reminded of outcomes;
- > Can also use to see trend changes over time – related to pain, swelling, other symptoms, or other responses tracked over time

Arriving or Not Arriving at Intended Locations #
Destination Checks detect arriving or not arriving at intended appointments, events or other destinations based on GPS readings;

At Hospital #
If care recipient has been at or near hospital for period of time (30+ minutes) (GPS-based)
When to use:
- As an additional way of keeping an eye on a care recipient;
- May prefer to disable if care recipient works or volunteers at hospital
Default setting:
- On; Looks for 30 minutes or more at hospital;

Engagement Level #
System detects low levels of user engagement
When to use:
Gives a sense of how much a care recipient is using their smartphone, responding to reminders and engaging with the system in general for that day
Default setting:
Always on; Care Alert Notifications for poor Engagement can be setup for particular times

Home All Day #
Detect if care recipient has not left house all day (5am to 5pm, GPS-based)
When to use:
- If generally not home all day, suggest keeping the feature enabled;
- If regularly home all day, disable this detection feature

Irregular Medical Device Readings #
Based on type of connected medical devices, LeapThru can report irregular measures in areas such as blood pressure or glucose readings;
Uses connected device(s) as provided by LeapThru health partners
When to use:
- To alert for irregular readings;
- To help facilitate adherence as directed by medical partner(s)

Lack of Movement #
Alerts for lack of movement detected over prolonged period of time during daytime (3+ hours);
Uses connected device such as smartwatch
When to use:
As an additional way of keeping an eye on a care recipient
Default setting:
- On with connected smartwatch;
- Ensure Active Hours were not changed to include nighttime hours if using this feature. If Active Hours include over night, may want to disable this feature

Low Battery #
Notified of care recipient’s low phone or watch battery or battery from other connected devices
When to use:
Get notified if care recipient’s smartphone or smartwatch battery is running low
Default setting:
On; set at 10% remaining battery by default
Low Level of Activity #
Periodically check throughout the day against expected levels of active movement personalized to care recipient’s situation
Uses connected device such as smartwatch
When to use:
Use to look for abnormal sedentary behavior or to see trend changes over time.
Default setting:
- On with following default step checks; care recipient does not meet:
- 500 steps by 10:00 am;
- 3500 steps by 2:00pm;
- 7000 steps by 6:00pm
- Suggest watching for 1 week and adjusting activity levels to align with expected behavior

Low or High Heart Rate Over a Period of Time #
Notifies for prolonged high or low heart rate during Active Hours time period (20 or more minutes based on readings);
Uses connected device such as smartwatch
When to use:
- For someone who regularly exercises, perhaps monitor only low thresholds;
- For someone who does not regularly exercise, set both low and high thresholds for potentially concerning situations
Default setting:
- On, with Low threshold set to 40 bps and high threshold set to 140 bps as default; customize to your situation;
- Suggest adjusting for individual

Medicine Adherence #
Help remind care recipient and monitor medication adherence;
Based on connected devices, can use either reminders or physical sensors from wireless pill boxes or medication dispensaries to determine adherence.
When to use:
- Reminders are not the most accurate way to gauge medication adherence. To more accurately gauge medication adherence, use a connected wireless pill box or medication dispenser provided by a LeapThru health partner. Review adherence on the Adherence Page of the LeapConnect™️ Portal.
Default setting:
- Medicine adherence is shown based on Medication Reminders by default; when connected to wireless pill box or medication dispenser, that becomes the source for medication adherence data.
