Care Alerts & Notifications are sent to permissioned family and other care team members for potentially concerning situations and other kinds of situations.  Care Alerts are triggered based on monitoring information from user smartphones and other connected devices (such as smartwatches or medical devices).

 


BreakThru™ allows specially permissioned family and care team members to cause the care recipient’s phone to automatically answer with the speakerphone enabled and camera turned on.  Caregivers can also remotely change between the care recipient’s front and back video cameras.

 


LeapThru helps provide insights into a loved one’s activity and behavior. The LeapConnect Web Portal has several different ways to better understand what might be going on.  The LeapConnect Dashboard gives up-to-date information at a glance.  The Timeline shows what has been going on during the day step-by-step.  LeapConnect’s Adherence page shows particular changes over time.

 


LeapThru uses everyday devices to help monitor and engage with a loved one through their smartphone, smartwatch and other connected devices.  In addition to monitoring device sensors for potential issues, users can remotely engage through: Automated care reminders, Medication reminders, Customizable check-ins, Care profile sharing, Specialized video calling, and BreakThru™ Connect.  Permissioned care team members can then be automatically notified of certain responses or outcomes.

 


LeapThru provides a number of ways for coordinating and communicating among family and other members of the care team, including: Integrated secure email, group chat, video calling and Care Alert related notes.  As well, when a care team member responds to a Care Alert, follow-up notifications are automatically sent to other care team members to help keep everyone up-to-date.

 


LeapThru can be customized for many different situations, including for those who are highly independent as well as for those who require more daily help.  LeapThru features can be switched on or off depending on user preferences.  And, each member of the care team has their own unique permissions and settings assigned for most features.  You can setup LeapThru once and let it run or regularly engage with it.
