Uses LeapConnect™ to help manage and monitor patients post-visit.

Providing patients needed information and actionable follow-ups to improve their care and success.

Dr. Randy Robinson, Chief Medical Officer, R-Health

“The logistics of caregiving can be overwhelming.

LeapThru® is designed to gather all available data into one platform and then distill the information into understandable, useful formats.

Patients and their families receive ongoing, personalized updates on wellness; caregivers gain insights into adherence and care effectiveness.”

ECHO Autism

Uses LeapConnect™ to help families of children with Autism connect with their care team and local community.

Dr. Rosanne Leipzig, Vice Chair for Education, Department of Geriatrics and Palliative Medicine Icahn School of Medicine at Mount Sinai

“LeapThru® approaches care giving differently – proactive rather than reactive.

The focus is on promoting prevention and adherence, actively engaging patients in their own health and identifying small issues before they become bigger medical concerns.

And it’s all connected through everyday technology.”

Connecting all the points of care is the starting point.

Many interactions take place every day with caregiving – taking medications, tracking mobility, interacting with caregivers and so much more. Exactly the reason we developed the LeapConnect™ Platform – technology to link these events via a single-source, closed-loop system. And managing all the data to create meaningful communications and prioritize care.

Better data means better care. Period.

From a clinical experience, we know well-informed patients have better outcomes. LeapThru makes it easier for individuals to make more consistent and smarter choices regarding their health. And for families to identify when they need to intervene, even from a distance.

Equally true for caregivers and insurers: through a system of monitoring, engagement and intervention, LeapThru is designed to improve care, optimize resource management and control costs.

Leading the innovation is LeapThru’s dedicated team of professionals.

Although backgrounds are diverse, our team and advisors all share a commitment to advancing home care.

“We didn’t start with the idea of developing a new technology platform. Rather we set out to address the human challenges presented by caregiving. We applied our healthcare knowledge; incorporated the perspective of patients, families, providers and insurers; and, took a deep dive into the current delivery approach. Only then did we start to think about how technology could enable – and enhance – caregiving.”
